If you have already enrolled and want to add a dependent to your health plan please contact firstname.lastname@example.org or the Administrator of your employee benefits plan. Members are unable to add dependents manually after enrollment, and this must be completed by Simply Benefits.
You may need to complete some forms to qualify the addition of a dependent outside of initial enrollment. Our team will help you with this process.
